Submitting this form authorizes Phil Weiss, Darshan Emeritus of Temple B'nai Brith, to act on your behalf and to sell, before the coming Passover, all your chametz as detailed below.

Phil will sell (or rent, for non-homeowners) the space where the chametz is found, and all the chametz that is located there will be sold (whether the space is owned or rented) for the duration of the holiday.

Submissions must be received by the end of the day on Tuesday, April 8.
